GPD arrest Chicago man for filling forged prescription

A Chicago man was arrested Tuesday night for possession of a controlled substance by deception, after allegedly bringing a forged prescription into a Galesburg Walgreens Pharmacy

According to Galesburg Police reports, officers were dispatched to the West Fremont Street Walgreens where pharmacy staff told police that on January 21, 2020, a fraudulent prescription for 30mg of Oxycodone was filled for a “Fred Henry.” Staff later learned it was a forged prescription.

A black male subject in the store on Tuesday night was identified as that same man, trying again to fill a forged prescription. Staff members presented police with the new prescription, which was nearly identical to the previously confirmed forged one.

The subject attempted to run from police in the store, despite officers warnings they would use tasers. He apparently tripped and fell landing on the pavement outside of the store and was apprehended. He was identified as 28-year-old Darren L. Cooks-Haynes.

Following Cooks-Haynes’ apprehension, officers continued their investigation into the supposed medical offices that made wrote out the prescription when Walgreens staff informed them that the company’s call center received a phone call from an unknown female on a restricted number. That woman stated, according to the report, “that her ‘cousin was arrested for picking up grandpa’s prescription” before demanding to be transferred’ to the Fremont Street Walgreens.

When the call center refused to transfer her, the report said that the woman became irate and “stated ‘if you don’t connect me, I am going to shoot up the Walgreens.”

Officers are also investigating into who made that call.
